New Patient Form


Our office, as well as the American Academy of Pediatric Dentistry, recommends that your child visit the dentist by his/her 1st birthday. You can make the first visit to the dentist enjoyable and positive. Your child should be informed of the visit and told that the dentist and their staff will explain all procedures and answer any questions. The less to-do concerning the visit, the better.

The first visit will be to examine and evaluate your child's dental health, growth and development, and to determine the nature of any dental problems.  Generally, diagnostic x-ray pictures will be taken which are a valuable adjunct for this evaluation.  We will not perform any treatment at this visit.  However, your child will receive a toothbrush kit and be shown
how to use it.

The findings of this examination will be discussed with you along with recommendations for care, prevention and estimated expenses involved, before any dentistry is begun.

Parents will be requested to remain in the Reception Room unless your child is very small or in situations where it is felt the parent could be of some assistance.

Your child's behavior is often influenced by your presence and if you are asked to accompany your child, please try to be as neutral as possible, and understand we are trying to establish a certain relationship with your child so he/she might accept care in the most cooperative manner.

Dentistry is a Health Service and not a game.  Our experience has been that most children will tolerate it well if it is explained to them.  It should not be compared to going to a circus or a party.  By avoiding such words as "hurt", "pain", "shot", "to pull a tooth", your child will not have any basis for being afraid of the dentist. Instead of saying "the doctor won't hurt you", say "he will be gentle with you."  Speak positively, but do not bribe.

Occasionally a child may cry, which is a normal reaction to fear or a strange environment.  Every effort will be made to familiarize your child with his/her new surroundings and usually, the presence of other children helps to alleviate any jitters.  We want your child to enjoy the visit.

The doctor will consult with you regarding his findings at the initial visit.  In order to provide your child with quality and efficient care, the doctor may not always be able to consult with you at subsequent treatment visits.  Also, unless there is a problem or you have a concern, the doctor may not always consult with you at the-recall check-up appointments.  Please feel free however, to request a consultation with the doctor any time your child comes to the office.

Our office has the permission from the Board of Education and the Board of Health to make appointments during school hours and as such should be considered an Excused Absence.  Please advise the receptionist if you need an excuse form.  When you are scheduling appointments for your children, please remember that a limited amount of after-school appointments are available.  It is impossible to accommodate everyone, so we ask that you please be flexible.
